Fractured Exhaled Nitric Oxide and Biologic Therapies for Paediatric Asthma

چکیده
Abstract Bronchial asthma is the most frequently diagnosed chronic respiratory disease in children. Treatment approaches should aim to achieve control, reduce limiting symptoms, and improve quality of life. Routine treatment patients with relies on assessment symptoms spirometry results. These diagnostic therapeutic strategies do not consider level inflammation airways as a fundamental pathognomonic feature disease. The use biomarkers increasing context efforts better understand individual pathways (asthma endotyping), potential for personalized innovative biologics. Elevated levels exhaled nitric oxide (FENO) represent an indirect marker T2 airways. FENO one few that have been applied routine clinical practice. High predict good response corticosteroids selected biologics (Omalizumab, Dupilumab, Mepolizumab, Tezepelumab), or increased risk exacerbation. this review evaluate advantages, disadvantages, applications test relation new options using asthma.

Nitric oxide (NO) is present in exhaled air of humans. This NO is mostly produced in the upper airways, whereas basal NO excretion in the lower airways is low. Children with Kartagener's syndrome have an almost total lack of NO in nasally derived air, whereas adult asthmatics have increased NO in orally exhaled air. NO excretion was measured in the nasal cavity and in orally exhaled air in 19 h...

Current recommendations are that children with asthma should be reviewed 6 monthly at minimum according to The National Heart, Lung and Blood Institute (NHLBI) guidelines in America (1), or annually according to The Scottish Intercollegiate Guideline Network (SIGN) (2) and the National Institute for Health and Care Excellence (NICE) (3) in the UK. NICE and SIGN suggest this review should includ...

and the lack of a response to hyperventilation, may not relate to asthma so much as they relate to disease severity or patient age. Finally, changes in responsiveness induced by allergen stimulation are not related to baseline AHR. This suggests that chronic persistent AHR may have underlying mechanisms that differ from those responsible for short-term changes in AHR induced by inflammation. Ou...
متن کاملExhaled nitric oxide and clinical phenotypes of childhood asthma
Whether exhaled NO helps to identify a specific phenotype of asthmatic patients remains debated. Our aims were to evaluate whether exhaled NO (FENO(0.05)) is independently associated (1) with underlying pathophysiological characteristics of asthma such as airway tone (bronchodilator response) and airway inflammation (inhaled corticosteroid [ICS]-dependant inflammation), and (2) with clinical ph...
